Translation of instructions

ENGLISH

This is the Health Record for your pregnancy. So we can give you the best care, you MUST bring this Record with you when you visit the hospital, or your doctor. Notes will be added to this Record at every visit.

It is your right to ask for an interpreter, or have someone ask for you.

This may be the only record kept, so you need to keep it with you all the time in a safe place (for example, in your handbag).

During your pregnancy please read this Record and ask any questions you may have. If you have any concerns between your visits, please phone your family doctor, obstetrician or midwife on the numbers shown on the front page of your Record.

If you wish, you will be given a photocopy of this Record at the end of your pregnancy. The original will be kept by the hospital.
